Health Center Registration
Registration is updated annually. All persons wishing to use any Center service, except WIC, must register as users of the Center by presenting income verification before services may be rendered. Hours for registration are:
- Monday, Wednesday, Thursday: 8:30 a.m. to 4:30 p.m.
- Tuesday: 8:30 a.m. to 6:30 p.m.
- Friday: 10:00 a.m. to 4:30 p.m.
Registration Requirements:
Documentation of eligibility for sliding fee schedule is verified by presenting one of the following:
Pharmacy hours: Mon., Wed., Thurs. 9:00 - 5:30; Tues. 9:00 - 6:30, Friday 10 - 5:00
On-Call Services - 24 Hours/Day
One of our physicians is on call 24 hours daily, 365 days a year. They may be reached by calling the Center Number - 769-3301.
